Before requesting our services, contact your family doctor or specialist to obtain the referral ticket. Present yourself at the hospital with the necessary documents.

Documents required for hospitalization

1. Referral ticket from the family doctor / Admission ticket from the specialist doctor.

Proof of the quality of the insured:

Employees

• Certificate from the Health Insurance Authority stating that the employing unit has paid up to date social health insurance.
• Certificate from the workplace with no. days C.M./ 12 months;

Retired:

• Pension coupon from the last 3 months (original or photocopy).

Unemployed:

• Unemployment certificate or photocopy of the unemployment card.

People without income:

• Certificate from the Health Insurance Authority, regarding the payment of the medical insurance or Co-insurance certificate.

Pupils under 18 and students:

• photocopy of the birth certificate (for children under 14) or identity card, certificate of pupil / student.

2. Identification card

Objects and materials required for hospitalization

In case of hospitalization, we recommend that you have the objects and materials necessary for your personal hygiene:

• Mugs and cutlery;

• Pajamas or nightgown;

• Slippers;

• Shaving cream and shaver ;

• Comb / hair brush;

• Towels, sponge and soap;

• Toothbrush and toothpaste;
