\u00a0 \u00a0Depending on the evolution of the patient's disease, the attending physician will determine
the date of discharge, which will be\ncommunicated to the patient or, in certain situations, to the
to the relative, one day before or at the latest in\nthe morning of the day of discharge.
\u00a0 The attending physician completes the Discharge Ticket and the Medical Letter in two copies;
one copy is attached to the Observation Sheet and the other is handed to the patient \/
the owner. The latter will send the Medical Letter to the Family Doctor.
\u00a0 The attending physician will record in the Discharge Note and in the Medical Letter the following:<\/strong>
-diagnosis of discharge;
-patient condition at discharge;
-medical treatment that the patient must follow;
-indications regarding the diet and lifestyle;
-date of next medical check-up.
\u00a0 Types of documents issued to the patient on discharge:<\/strong>
\u2013 Discharge ticket;
\u2013 Medical Letter;
\u2013 Medical leave certificate (as applicable);
\u2013 Medical certificate (as appropriate);
\u2013 Compensated or simple prescription (as appropriate);
\u2013 Referral tickets to other specialists or for various paraclinical investigations (after
case);
\u2013 Medical report to the expertise services (as the case may be);
If the patient is discharged on request, the risks to which he may be exposed are explained. In the case of the minor patient, the risks are explained to the relative.
